Our goal at VIDA Integrative Medicine is to provide you with a personalized level of care. We are committed to working with you and creating a partnership so that you may achieve optimal health.
It is important for you to read the information and complete all forms that are sent to you prior to your initial office visit. All forms should be completed and mailed, scanned or faxed to our office 5 days prior to your appointment. This will allow us to thoroughly review your information prior to our appointment in order to maximize the quality and efficiency of your visit.
VIDA Integrative Medicine operates as a self-pay office. Payment is expected at time of service.
Our office accepts cash, checks or credit cards for services rendered. When you schedule an initial visit, we request a credit card to be placed on file to hold the appointment for you. No charges will be applied to your credit card until the day of your appointment unless you miss or cancel an appointment without proper notice.
On the day of your scheduled appointment, all charges for consultations (medical and nutritional) as well as laboratory testing will be itemized and reviewed with you. Payment is due on the day of service.
Follow-up phone consultations will be billed to your credit card on file in our office unless you provide other payment information and instructions prior to your appointment. If additional lab tests are required and our office sends test kits, the appropriate fees will be charged to your account.
Insurance – Why we do not take insurance
VIDA Integrative Medicine does not accept insurance, Medicare or Medicaid. In some cases your services (office visits, phone consultations or lab tests) will be reimbursed by insurance, but we cannot ensure that. Please check with your insurance company prior to your visit. Our office does not bill your insurance directly but we will provide you with diagnosis and procedure codes to assist you with possible insurance reimbursement.
For you to enjoy all the benefits of concierge medicine without upfront fees and contracts, we do not bill insurance companies directly for your care. This involves much time and staffing, reducing the attention you receive during and after your office visit. We feel that the health decisions that need to be made should be between you and your provider, not your insurance company. We feel it is important that we give you the time necessary to full understand your issues and delve into the root cause, educating you and supporting you throughout the process. This cannot be adequately done in 10-minute bites of time. Your health goals are our focus, not the insurance company’s bottom line. While we understand you may choose to do or not do something based on the cost that is a separate decision from what is best for your health.
Although we require payment from you at the time of service, we will do everything possible to help you get reimbursed for at least a portion of your visit. You will be provided with a comprehensive bill that includes all the codes for treatment and the amount you paid. This should be submitted to your insurance company for reimbursement. Most insurance companies will pay you back 60-80% of the cost of your visit. Call your insurance company to find out your deductible and out of network coverage.
If you have insurance, any referrals, many lab tests, and radiology exams will be billed directly to your insurance company. The primary out-of-pocket expense for you will be the cost of our office visit. There can also be additional costs for specialty lab tests, EKGs, medications, vitamins, supplements or B12/B complex shots given during your office visit.
VIDA offers a 4 Month Wellness Package, in which you receive four visits over four months at a discounted rate. A down payment is made at the first visit, then the rest of the package can be paid for in two equal monthly payments for your convenience.
Medical Records may only be released with your authorization. A medical records release form will be emailed to you prior to your visit. You are responsible for obtaining previous medical records. Please contact your physician to obtain these records.

Confirmation and Cancellation of Appointments
Due to the personalized nature of our consultations, there is a 48 hour cancellation policy. You may cancel your appointment by calling our office. If calling after hours, please leave a message.
Prescription Refill Request
It may take up to 72 hours to process a prescription refill. Please plan ahead to avoid any interruption in your medications. Prescription refills can be faxed to our office by your pharmacy. Our fax number is 954-448-7741.
